As a North Dakota Cancer Coalition member you have easy access to state and national resources for use at the local level.
If you are not a member of the North Dakota Cancer Coalition and are interested in receiving resources simply fill out a North Dakota Cancer Coalition application form and the resources will be at your finger tips!
Resources Guidelines
Please submit NDCC Resource Order Form 2 weeks prior to the date resources are needed.

Resource Evaluation Form
Due to requirements from the Centers for Disease Prevention and Control we must receive a brief evaluation from all individuals/organizations using resources provided by the North Dakota Cancer Coalition and the Comprehensive Cancer Control Program. Please complete the attached form and e-mail to Candy Getz, Membership/Resource Committee at cgetz@nd.gov following the use of all resources.
